It is our preference that you use the on-line registration process rather than mailing in the paper forms.  However, the paper form is still acceptable, and may also be used as a means for gathering information prior to completing the on-line registration process.  If you use the paper based form, a copy of the player roster must be type written, and sent electronically.  Paper based application forms should be mailed to:                      North American Hockey
